美國醫師執照的發給是各州政府的權限,因此各州規定略有不同基本要求是通過USMLE所有的step(1,2CK,2CS,3)加上1到3年的住院醫師訓練兩者缺一不可(有些州還加上另外的要求)因此即使不想考專科醫師,仍然要接受住院醫師訓練,否則拿不到執照我這裡所說的執照是指可以獨立行醫,開業或受聘為主治醫師的資格性質上或許類似於我國的執業執照但是個人覺得名稱不重要,
反正就是實質上的行醫資格外國和本國畢業生考的是相同的USMLE(美國醫師執照考試)但是負責報名事務與資格審核的單位不同,住院醫師訓練資歷的要求也不同
有人(特指小波波)因為考相同的試(USMLE)就以為美國對本國與外國畢業生一視同仁,其實是相當大的誤解
ECFMG是負責外國醫學院畢業生(IMG,International Medical Graduate)資格審核和USMLE step1,2CK,2CS報名事務的單位該醫學院必須名列在IMED中(International Medical Education Directory)他們只看醫學院的訓練,不承認畢業後的一切資歷如住院醫師訓練,
外國醫師執照等IMG必須先通過USMLE step1,2CK,2CS,由ECFMG發給ECFMG Certificate才可以申請住院醫師
住院醫師(IMG)要2到3年之後才會發給執業執照
本國畢業生只要畢業就可申請,考試可以之後慢慢考,有點像台灣國考還沒過也可以先當RUSMLE step3要向各州政府報名,因為各州規定不同,所以又造成了一些誤解大部分的州規定要當完一年住院醫師才能報考,有十幾個州則無此要求其實此處州政府只是受理報名的單位與考生將在何處當住院醫師或未來要在何處執業無關,也與想在何處考試無關簡單的說,考生可以隨便向任一州政府報名(當然如果資格不符會被打回票)報名核准後再選擇美國境內任一考試中心應試
有人以為某些州不用當R,直接考完USMLE step3就可以拿到執照是很嚴重的誤解
關於取得醫師執照前,住院醫師訓練資歷的要求絕大部分的州對IMG和本國畢業生有不同的標準通常是本國生1-2年,IMG 2-3年因此制度上,美國對本國和外國醫學院畢業生並非一視同仁只是其差別並非在考試本身,而是在考試以外的部分(申請住院醫師的資格要求與訓練資歷)遑論實務上,IMG申請住院醫師的錄取率不到本國生的一半
這也是一個重要的篩選關卡補充一點題外話,IMED上的確可以查到波大國際班,當然ECFMG仍有最後資格審核的權力其實我很希望自認為實力堅強又有國際觀的波波們能去考個USMLE(USD800的報名費他們應該不是付不起)只要有十分之一的人能考到245/99以上,我願意免費替他們宣傳"波波真的實力堅強" !
I'm currently working as an internal medicine resident in United states. I believed you do not understand the usmle system entirely. When I was the second year medical student, I was eligible to take USMLE step 1 and anytime during the 3rd and 4th year of medical school, medical students were able to take USMLE step 2. In order to receive the medical license, step 3 has to be taken during the internship year. In comparison, I believe the step 1 and 2 are almost equivalent to 國考 United states did not set up different set of exam specifically for foreign graduates. If you would like to compare everything to United states, then everyone, no matter where you graduate from, should be able to take the same exam. The Ministry of Exam can set a higher standard for passing to ensure the "real" quality of Taiwanese health.
回覆刪除USMLE itself didn't differ. But if you are a FMG(IMG), it's more difficult to enter any program than AMG(citizen or green card holder). USMLE itself is easy to pass, but that doesn't mean it's easy to be a real practicing doctor in the United States.
